Canada Self-Attestation for Suppliers

Revisions allow you to track differences between multiple versions of your content, and revert back to older versions.

Suppliers can use this attestation to demonstrate compliance to the terms of the US/Canada Equivalence for their buyers.

This form cannot be used by operations directly exporting to Canada. Certifier verification is now required for all product entering Canada. CCOF operations must enroll in the Global Market Access (GMA) program for US/Canada equivalence to obtain certifier verificaion. Visit our Canada page for additional information on the equivalence and who must enroll in the GMA program.
