| Expedited/Rush Organic Certification Program- Use to contract expedited/rush services for highly time sensitive new certifications and add acreages or new product lines requiring inspections. |

Certification Renewal/Continuation of Certification Contract
CCOF also provides an online certification renewal. Use this form to renew your annual CCOF certification and report OSP updates as needed. Must be completed annually to continue certification and remain in good standing. Simply complete blanks, print and send to CCOF. May also be used to withdraw from CCOF certification. |

| Storage Facility Affidavit- Use to verify organic
handling practices at storage facilities that are not required to
be certified or inspected under the NOP. Examples include coolers,
warehouses and distribution centers. |

Sample Land Use Affidavit- Use to establish land use history by previous mangers/owners of new parcels being brought into the CCOF program.

| Transitional Status Request Form- Use to request transitional certification status for CCOF operations and/or particular parcels. Must completed and approved prior to use of CCOF Certified Transitional logo or claims. |

| Add traded/brokered/private labeled product (OSP 2.5)- Used by CCOF certified brokers, traders, or certified private labelers to add new products to their certification. Applicable where products are not re-packaged or processed while in the CCOF operator's possession. |

| Private Label Co-Packer Profile (OSP-H2.6)- For CCOF co-packers to identify private label products and brands they produce. Identifies if the Name and Seal Use Agreement (below) is required. |

| CCOF Name & Seal Use Agreement (Private Label Affidavit)- Used for approval of products
packaged into brands that are packaged by CCOF certified entities and identify CCOF as the certifier but the owner of the brand does not
carry CCOF certification. |

Non-Organic Product Declaration- Used by CCOF clients to document non-GMO, sewage sludge, and irradiation compliance for non-organic ingredients, citric acid, flavors, etc.
