Affidavit of Input Material Composition

This form may be used by CCOF-certified farm operations for non-manufactured input materials when the operation requesting use can provide an attestation regarding the composition. For example, straw, wood chips, raw manure, and on-farm made preparations.

Biodiversity & Natural Resource Cost-Based Monitoring Log

Organic production must, by definition, be managed to respond to site-specific conditions by integrating cultural, biological and mechanical practices that foster cycling of resources, promote ecological balance and conserve natural resources. Certified organic operations must select, carry out and record production practices that “maintain or improve the natural resources of the operation,” (NOP§205.200). This template can be used to track costs associated with pest and fertility management on a daily, weekly, monthly or annual basis in order to monitor the effectiveness of the Biodiversity Conservation & Natural Resource Management plan outlined in OSP form G4.2 

This template is available for download as a PDF or Microsoft Word document. If you do not have Microsoft Office you can save the file in your Google Drive and edit there. A Google email account is required.
